Medical Release & Waiver of Liability

I attest that the camp participant named above is in good physical health.  I hereby acknowledge that participation in the camp and its related activities carries an inherent risk of services physical or mental injury. I hereby assume the sole responsibility for all such risks of injury by my signature below, and release Darrell Freeman and the Craig McAlister Explosive Athletic Camp, any affiliated companies, staff, and agents thereof, from any and all liability arising from injuries sustained by the above named child’s participation in the camp, to include the failure of any equipment or defects in the premises.  I hereby authorize any medical treatment necessary for the child listed above, incurred while participating in the camp, as determined by a competent medical authority, hospital, or professional practitioner.
